Attaching patch.  Out-of-the-box at least, Sphinx seems to have the constraint that the "home page" (what the link in the upper-left corner points to) needs to be the same as the table of contents (what the "table of contents" link in the left column points to).

Retaining the same home page seems to be the most important, so for now at least, I'm proposing putting the table of contents at the bottom of the home page (with the home page itself not part of the contents).

I also re-did the header formatting for a few pages to make the nesting levels match the page list.
