My understanding is that David noticed the problem originally on MacOS.  If the emulation is indeed being used on that platform (and a little googling indicates the MacOS posix semaphore implementation is considered at least slightly broken, and FreeBSD didn't support it until 7.2), then perhaps that is why he was looking at that code.
